Please make sure to inform the Personnel Office any time you
experience a life changing event such as a marriage, divorce,
adoption, birth of a child, death of a spouse or beneficiary.
Your information should be updated as soon as possible after the event
occurs. Please note that changes to your beneficiary for
Teachers/Public School Retirement should be reported to the
Personnel Office and changes to beneficiaries for insurance should
be reported to the Benefits Office.
Completed forms may be forwarded
to the Personnel Office through the Courier or faxed to
431-1263. Changes will be processed upon receipt of
completed forms.
